Dear Parent/Guardian,

As per WRDSB policy, POC’s will be distributed this week (Sept 13 – 17), to all students who are reported to have any type of life threatening health condition.

If your student does not bring home their POC by Thursday Sept 16, 2021, please notify the school immediately at 519-884-9590.

If your student has developed a new health condition or you are not sure if your student requires a POC, please contact the school immediately at 519-884-9590.

Examples of life threatening health conditions requiring a POC are:

  • Anaphylaxis (allergies requiring an EpiPen)
  • Asthma
  • Diabetes 
  • Epilepsy
  • Other (any other potential health condition that is considered to be life threatening)
