Medical Plan of Care

As of 2018, The Ministry of Education directed school boards to provide a plan of care for all students experiencing a Prevalent Medical Condition as follows:

    • Anaphylaxis
    • Asthma
    • Epilepsy and Seizure disorders
    • Diabetes
    • Other conditions

Board Policy 1006 was created to promote the safety and wellbeing of students, to establish and maintain administrative procedures surrounding the needs of students experiencing one or more of the prevalent medical conditions listed above.

The Plan of Care highlights any life-saving interventions required for your child in the event of a medical emergency.

 

What Parents/Guardians Need to do?

  1. If your child already has a medical plan of care from the academic year 2019/20, please use the following form to update us with changes that need to be made to that plan of care Update Plan of Care
  2. If your child requires a New Plan of Care, please complete the form: New Plan of Care Once received, a specific form relevant to the medical condition will be sent to you for completion. From this information, a draft will be generated.
  3. If you are not sure about whether a Plan of Care is necessary for your child please speak with your family doctor.
