If you have reached your 40 HOURS OF COMMUNITY SERVICE HOURS, please submit your hours in the Google Form below. Please ONLY enter your hours IF you have 40 hours.

Do you have less than 40 community service hours?

If you have less than 40 hours, please keep a record of your hours and ONLY submit your hours once you reach your 40 hours.
