Attention Parents/Guardians and students 18 and over, 
Data Verification Forms are a critical document which ensures the contact information for your child/children or yourself is correct. Special attention needs to be given to the phone number and email address. Please take some to thoroughly review the Data Verification Form and return as soon as possible.
 
Below are the timelines for receiving the Data Verification Form for each cohort:
  • Cohort A – October 1st
  • Cohort B – Sept 24th and 25th
  • Cohort C and D (DLP)
    • Forms will be mailed out Sept 25th
